The battle that Rush is in is now not just being above other doctors or having the better cure, but just a cure for himself.Rush has gotten yet another case of the Yellow Fever, and seemed to be doing nothing about it. The story begins by explaining how the public sewer and water system in Philadelphia in the late 1700s could lead to such an outbreak. Since Philadelphia was the temporary capital of our young nation the epidemic all but shut down the government. That cure was through humor and denial.

Murphy also describes the interplay between Philadelphia and neighboring areas, and the conflict between the ordinary person and those who held power/made decisions.
